The gut–skin axis: how the gut talks to your skin, how far

The gut–skin axis is the two-way link between the gut microbiome and the skin. Gut bacteria shape the immune cells and metabolites that circulate to the skin; the skin's own microbes respond to that immune tone. Eczema, acne and rosacea are associated with gut changes — through inflammation, barrier function and diet, not through bacteria travelling to the face.

What this system is

Skin and gut are both borders. Both carry a microbial community, both sit on a thin barrier, and both are watched by the same immune system. What connects them is not a tube but circulation: immune cells trained in the gut lining travel; metabolites made by gut bacteria — short-chain fatty acids, bile-acid derivatives, tryptophan products — enter the blood and reach the skin, where they alter how skin cells and resident immune cells behave.

The traffic runs both ways. Ultraviolet light on the skin changes the gut community in animal studies, and stress hormones alter both.

The skin has its own microbiome, dominated on the face by Cutibacterium and Staphylococcus. That community is shaped by sebum and pH — and sebum and pH are influenced by diet and inflammation, which is where the gut enters.

The three questions people ask

Does gut health cause acne? Acne is a disease of the follicle: sebum, keratin and a particular strain profile of Cutibacterium acnes. Diet — high-glycaemic foods, dairy in some studies — tracks with acne, and diet also shapes the gut. The gut is a plausible route, not a proven cause.

Can probiotics help eczema? For infants at high risk, certain strains given to mother and child track with less eczema in some trials; for existing eczema in adults, results are inconsistent.

Is rosacea a gut problem? Bacterial overgrowth in the small intestine is reported more often in people with rosacea, and treating it has cleared skin in small case series. It is an observation, not a rule.

What the science can and cannot say yet

It can say: gut microbes shape systemic immune tone; that tone shows in the skin; diet influences both ends; in infants, early microbial exposure tracks with allergy risk.

It cannot yet say: which gut change causes which skin disease, whether an adult with acne or eczema should treat their gut, or what a stool test would tell a dermatologist. Human evidence is observational and small.
